The B-Complex Family: Every B Vitamin, What It Does, and When It Helps

Why the B-Complex Family Matters

The B-complex family includes eight water-soluble vitamins: B1, B2, B3, B5, B6, B7, B9, and B12. These nutrients work primarily as coenzymes, which means they help enzymes perform essential biochemical reactions throughout the body.

Together, B vitamins support carbohydrate, fat, and protein metabolism. They help transform food into usable cellular energy, maintain neurological function, support red blood cell production, and participate in DNA synthesis. They also contribute to healthy skin, muscle function, immune activity, and cardiovascular physiology.

A deficiency in one B vitamin may present as fatigue, weakness, numbness, anemia, impaired concentration, or changes in skin and mucous membranes. However, these symptoms have many possible causes. Clinical evaluation and appropriate laboratory testing are important before beginning high-dose supplementation.

B vitamins are found in many whole foods, including legumes, leafy greens, eggs, fish, poultry, dairy products, whole grains, nuts, and seeds. A balanced diet is the appropriate starting point for most wellness plans. Supplementation becomes more relevant when dietary intake is inadequate, absorption is impaired, requirements are increased, or a deficiency has been identified.

Vitamin B1: Thiamine

What it is and how it helps: Thiamine is converted into thiamine pyrophosphate, a coenzyme required for carbohydrate metabolism and the breakdown of certain amino acids. It helps the body process glucose and supports the production of adenosine triphosphate, or ATP, the primary energy currency of cells. The nervous system and cardiac muscle are particularly dependent on adequate thiamine because both tissues require a continuous energy supply. Thiamine also supports normal nerve signal transmission and muscular coordination.

Whole-food sources: Pork, fish, beans and lentils, sunflower seeds, whole and enriched grains, brown rice, potatoes, and fortified cereals.

Clinical example: Thiamine is used clinically when deficiency is suspected or confirmed, including in severe malnutrition, chronic alcohol-use disorder, prolonged vomiting, or certain gastrointestinal conditions. Severe deficiency can cause beriberi, which may involve peripheral neuropathy or cardiovascular dysfunction, as well as Wernicke-Korsakoff syndrome.

Vitamin B2: Riboflavin

What it is and how it helps: Riboflavin forms the coenzymes FAD and FMN, which participate in oxidation-reduction reactions that help release energy from carbohydrates, fatty acids, and amino acids. It also supports cellular antioxidant systems, iron metabolism, skin integrity, and normal red blood cell production.

Whole-food sources: Milk, yogurt, and other dairy products, eggs, lean meat, liver, almonds, mushrooms, fortified cereals, and enriched grains.

Clinical example: Riboflavin is used to correct deficiency and is also used in some evidence-based migraine-prevention protocols. The dose and duration should be determined with a qualified healthcare professional.

Vitamin B3: Niacin

What it is and how it helps: Niacin includes nicotinic acid and nicotinamide. It is incorporated into NAD and NADP, two coenzymes involved in energy production, DNA repair, fatty acid synthesis, and cellular antioxidant reactions. It supports metabolism, skin health, the digestive tract, and the nervous system.

Whole-food sources: Poultry, beef, tuna and salmon, peanuts, seeds, mushrooms, whole grains, and fortified breads and cereals.

Clinical example: Clinicians use niacin to treat pellagra. Prescription-strength niacin has also been used in lipid management. It is not appropriate to use high-dose over-the-counter niacin for cholesterol control without medical supervision because it may cause flushing, liver injury, and elevated blood glucose.

Vitamin B5: Pantothenic Acid

What it is and how it helps: Pantothenic acid is a component of coenzyme A, required for fatty acid metabolism and acyl-transfer reactions. It helps the body use macronutrients and supports the synthesis of cholesterol, steroid hormones, and certain neurotransmitters. Clinically significant deficiency is uncommon.

Whole-food sources: Chicken and beef, organ meats, mushrooms, avocado, potatoes, lentils, peanuts, whole grains, yogurt, and eggs.

Clinical example: Supplementation is generally reserved for confirmed deficiency. Severe deficiency may produce fatigue, irritability, gastrointestinal distress, and burning sensations, which require professional evaluation.

Vitamin B6: Pyridoxine

What it is and how it helps: Active forms of vitamin B6 act as coenzymes in amino acid metabolism, neurotransmitter synthesis (serotonin, dopamine, GABA), glycogen breakdown, hemoglobin formation, and homocysteine metabolism.

Whole-food sources: Chickpeas, poultry, fish, beef, potatoes, bananas, fortified cereals, and winter squash.

Clinical example: Vitamin B6 may be used to treat deficiency or prevent neuropathy associated with certain medications. It is also used under medical guidance for nausea during pregnancy. Chronic high-dose B6 supplementation can cause peripheral neuropathy, so high-dose use without oversight should be avoided.

Vitamin B7: Biotin

What it is and how it helps: Biotin acts as a coenzyme for carboxylase enzymes involved in fatty acid synthesis, gluconeogenesis, and amino acid metabolism. While promoted for hair, skin, and nails, deficiency is rare in those consuming a balanced diet.

Whole-food sources: Cooked eggs, salmon, liver, nuts and seeds, legumes, sweet potatoes, cauliflower, and whole grains.

Clinical example: Biotin is used to treat confirmed deficiency. High-dose biotin can interfere with laboratory tests, including thyroid and cardiac marker tests, so patients should inform their clinician before testing.

Vitamin B9: Folate

What it is and how it helps: Folate (naturally occurring) and folic acid (synthetic) transfer one-carbon units required for DNA and RNA synthesis, cell division, amino acid metabolism, and methylation. It is critical for rapidly dividing tissues and works closely with vitamin B12.

Whole-food sources: Spinach and leafy greens, asparagus, broccoli, beans and lentils, avocado, citrus fruits, liver, and fortified grains.

Clinical example: Folate supplementation is standard in preconception and early-pregnancy care to reduce neural tube defect risks. It should not be used to mask unexplained anemia without evaluating vitamin B12 status.

Vitamin B12: Cobalamin

What it is and how it helps: Vitamin B12 is required for DNA synthesis, red blood cell formation, fatty acid metabolism, and maintenance of myelin sheath integrity around nerve fibers. The liver stores substantial amounts, but deficiency can develop gradually when absorption is impaired.

Whole-food sources: Fish and shellfish, beef, poultry, eggs, milk, yogurt, and liver. Strict vegans generally require fortified foods or supplementation.

Clinical example: B12 is used to treat dietary deficiency, pernicious anemia, and malabsorption. Low B12 can cause megaloblastic anemia, numbness, tingling, balance problems, and peripheral neuropathy.
