Creatine Explained: What It Is, How It Works, and Who Should Use It
For athletes, active adults, and people beginning a strength-training program in San Jose, creatine is one of the most frequently discussed supplements. It is also one of the most researched. However, creatine is not a magic pill, a steroid, or a substitute for sleep, nutrition, rehabilitation, or a well-designed exercise program.
Creatine is a naturally occurring compound that supports cellular energy production. When used appropriately by healthy adults, it can improve repeated high-intensity performance and increase the training-related development of strength and lean body mass. It also requires caution in people with kidney disease, abnormal kidney laboratory results, or other significant health conditions.
What Is Creatine?
Creatine is a nitrogen-containing compound produced naturally from the amino acids arginine, glycine, and methionine. The body synthesizes creatine primarily through the kidneys, liver, and pancreas. Additional creatine comes from dietary sources such as meat and fish.
Approximately 95 percent of the body’s creatine is stored in skeletal muscle. The remainder is found in tissues with high energy demands, including the brain and heart. Inside muscle cells, creatine exists as free creatine and phosphocreatine.
The most studied supplemental form is **creatine monohydrate**. Other forms are marketed extensively, but they have not demonstrated consistent superiority over monohydrate in effectiveness, safety, or value.
How Does Creatine Work in the Body?
Muscle contraction requires adenosine triphosphate, commonly called ATP. ATP is the immediate energy currency used by muscle fibers. When ATP releases energy, it becomes adenosine diphosphate, or ADP.
The body cannot store large quantities of ATP for immediate use. Instead, phosphocreatine functions as a rapid energy reserve. Through the creatine kinase reaction, phosphocreatine donates a phosphate group to ADP and helps regenerate ATP.
This system is especially important during brief, demanding activities such as:
- Heavy resistance training
- Sprinting
- Jumping
- Repeated changes of direction
- High-intensity interval training
- Short bursts during basketball, soccer, hockey, and other field sports
Supplementation increases the amount of creatine available in muscle cells. Once muscle stores become saturated, the phosphocreatine system can support greater repeated effort. This may allow a person to complete an additional repetition, maintain power across several sprints, or tolerate more total training volume.
Creatine does not create energy from nothing. It helps the body recycle energy more rapidly during activities that depend heavily on the phosphagen system.
Why Is Creatine Suggested?
The strongest evidence supports creatine for improving high-intensity exercise capacity, strength development, and lean body mass when it is combined with resistance training.
A 2024 systematic review and meta-analysis of randomized controlled trials found that creatine combined with resistance training improved upper- and lower-body strength in healthy adults under age 50 compared with resistance training alone. The same year, another meta-analysis found that creatine combined with resistance training increased lean body mass by approximately 1.14 kilograms on average compared with training without creatine.
These are meaningful but moderate effects. Individual responses vary. Some people experience a more noticeable improvement, while others respond minimally. The greatest benefits occur when creatine is paired with progressive resistance training, adequate protein and calorie intake, proper hydration, and sufficient recovery.
Creatine may also be useful for older adults who are attempting to preserve muscle strength and functional capacity. Research in older populations shows that creatine combined with resistance exercise can improve lean tissue mass and strength. This is relevant because age-related loss of muscle, called sarcopenia, can affect balance, mobility, independence, and recovery from injury.
Creatine has also been studied for cognitive function, sleep deprivation, glucose regulation, rehabilitation, and certain neurological conditions. These applications are promising in some areas, but the evidence is not as consistent as it is for strength and repeated high-intensity performance. We do not recommend treating creatine as a cure for a medical condition.
Who Should Consider Creatine?
Creatine may be reasonable for healthy adults who:
- Perform regular resistance training
- Participate in sprinting, power, or repeated high-intensity sports
- Want to improve training capacity, strength, or lean body mass
- Are older adults participating in a safe, supervised strength program
- Consume little or no meat and may have lower baseline muscle creatine stores
Vegetarians and vegans may respond particularly well because dietary creatine is concentrated in animal foods. However, response still depends on training quality, total nutrition, sleep, and individual physiology.
Creatine should not be started without medical guidance by people with:
- Known kidney disease or reduced kidney function
- Abnormal creatinine, estimated glomerular filtration rate, or urine protein results
- A solitary kidney or a history of kidney injury
- Significant diabetes or hypertension involving the kidneys
- Multiple medications that may affect kidney function
- Pregnancy or breastfeeding
- A complex chronic, metabolic, inflammatory, or neurological disorder
Children and adolescents should use supplements only under the direction of a qualified pediatrician, sports-medicine physician, or registered dietitian familiar with the individual’s health history.

When Should You Take Creatine?
Creatine works by gradually increasing muscle stores. It does not provide an immediate stimulant effect like caffeine. For this reason, consistency is more important than taking it at a precise time before or after exercise.
A common maintenance protocol for healthy adults is:
- 3 to 5 grams of creatine monohydrate once daily
- Taken with water, a meal, or a recovery shake
- Continued consistently on training and rest days
A loading phase is optional. Some protocols use approximately 20 grams daily, divided into four smaller doses, for five to seven days, followed by 3 to 5 grams daily. Loading saturates muscle stores more rapidly, but it is not necessary. It also increases the chance of bloating, diarrhea, or stomach discomfort.
There is no established need to cycle creatine on and off when a healthy adult uses standard maintenance dosing. Choose a product from a reputable manufacturer with quality testing, and avoid combining multiple products that each contain creatine.
Can You Take Too Much Creatine?
Yes. Although creatine monohydrate has a strong safety record at standard doses, more is not automatically better.
There is no clearly established acute lethal dose for ordinary oral creatine monohydrate, but unlimited use is not considered appropriate. Excessive intake can cause:
- Nausea, diarrhea, cramping, or abdominal discomfort
- Increased water retention and temporary weight gain
- Difficulty distinguishing normal supplement-related weight changes from unwanted changes
- A rise in blood creatinine that can complicate interpretation of kidney laboratory results
- Greater concern when combined with dehydration, heat stress, kidney disease, or medications that affect renal function
Creatine itself is converted into creatinine, a normal waste product measured in many kidney tests. Supplementation can modestly increase serum creatinine without proving that kidney filtration has declined. However, an abnormal result must never be dismissed automatically. Tell your physician that you use creatine before blood or urine testing so the result can be interpreted properly. Additional tests, such as cystatin C or a directly measured filtration assessment, may sometimes be appropriate.

What Does the Research Show?
Creatine is supported by decades of research. Important evidence includes:
- The 2017 International Society of Sports Nutrition position stand concluded that creatine monohydrate improves high-intensity exercise capacity and lean body mass and is generally safe for healthy individuals when used according to established protocols. Read the position stand.
- A 2021 review addressing common creatine misconceptions found no consistent evidence that recommended dosing causes dehydration, muscle cramping, or kidney damage in healthy individuals. Review article.

The appropriate conclusion is balanced: creatine is effective for specific exercise goals and generally well tolerated by healthy adults at recommended doses. It is not necessary for everyone, and it is not proven to treat every condition promoted online.
A Safe, Practical Summary
For a healthy adult considering creatine, the most conservative approach is to discuss current medications, health history, and recent kidney laboratory results with a physician first. If appropriate, use plain creatine monohydrate at 3 to 5 grams daily, maintain normal hydration, and focus on progressive exercise rather than excessive dosing.
